The following student forms are available for current students. Please use these forms to submit student requests. Prospective Students will find application forms on the Admissions page.
Major and Minor Application Forms - These forms allow current students to apply for majors and declare minors.
"Drop" Major Form - This form allows current students to drop a major.
Incomplete (Grade of I) Request - These forms allow current students to request a grade of Incomplete (I) from their professors.

Records Release Form - This form allows current students to release educational records to other entities, internal and/or external.

Transient Study Form - This form allows current students to request permission to enroll in a course in a different institution than Creighton University. Study abroad arranged through the International Programs office does not require the use of this form.
