Obtaining or Changing a P-Card

Complete the Credit Card Action Request Form to:

  • Request a new card
  • Request lost or stolen card replacement
  • Change Custodian
  • Close Account
  • Temporary or permanent increase to monthly credit limit

Lost or Stolen Card

To report a lost or stolen card during normal Purchasing Department business hours -- call 280-2712.

To report a lost or stolen card after normal Purchasing Department business hours -- call 800-688-7070. The bank representative contacted after hours may not be familiar with Creighton’s business account. The only information necessary for a Creighton business account is the last four digits of the cardholder’s social security number or numeric portion of the cardholder's NetID and our billing address of PO Box 3266, Omaha, NE 68103-0266. No other information is required.
