Credential Files
Credential file fees are $5.00 per request. Files will be sent after payment is received in the certification office.
Please include the following in your request for credential files:
1. Your name (maiden name if applicable), address and phone number.
2. Add your social security number (if you graduated in 1992 or after) or Date of birth (if you graduated in 1991 or before).
3. Number of files to be sent.
4. School addresses and contact names for each request.
5. Specify which recommendations/documents to be included in the file.
6. Include any updates or changes to be made before the file is sent.

NOTE:
Credential files for current Creighton Student Teachers: $25.00 for initial credential file, valid for 1 year (includes set up, maintenance, materials, copying, postage and 7 credential file requests).
Credential file requests after 1 year and/or after the first 7 requests are $5.00 per request.)
Graduates, Alumni, Certification only $5.00 per credential file request.
