The Field Practicum Agency Agreement is a contract between the practicum agency, the Creighton University Social Work Student, and the Creighton University Social Work Program. This agreement must be completed prior to the student beginning work at the practicum site. It should be submitted electronically to the Program Field Practicum Supervisor, Dr. Barb Harris.
