IMMUNIZATION REQUIREMENTS

Vaccination data must be on file  BEFORE August 1st for New Students & Returning Students!  

Students registered in a Health Science program (Dentistry, EMS, Medicine, Nursing, Occupational Therapy, Pharmacy, Physical Therapy) must complete All Health Science Student requirements, except for seasonal flu vaccine, BEFORE classes begin unless instructed otherwise by the school. 

Submit vaccination records via NEST; fax—402-280-1859; or send a copy to the Center for Health and Counseling—2500 California Plaza-Omaha, NE  68178.

New Information on Meningitis Vaccine-Fall 2012: 
CDC recommends students receive a booster dose of meningitis before after age 16 to assure protection during the college years.  This vaccine is required for students living in Creighton University residence halls or apartments.  Please contact Student Health Services if you have questions regarding this new requirement.
