Attention Deficit Hyperactivity Disorder (ADHD)

 

The Center for Health and Counseling provides evaluation and management for students with Attention Deficit Hyperactivity Disorder (ADHD).  In order to begin medication management at Student Health Services, the following must occur:

Previously diagnosed at another office:

1. Fax a copy of previous records with diagnostic testing to:

Creighton Student Counseling Services (fax: 402-280-1859).

2. Previous records will be reviewed and if documentation is sufficient to support the diagnosis of ADHD, Counseling Services will forward a recommendation to Student Health Services.  You will be notified by Student Counseling Services after the records have been reviewed.

3. If records are deemed insufficient and re-evaluation is needed, the student will be notified to schedule an appointment with Student Counseling Services. The fee for re-evaluation is $200. Results of the evaluation will be forwarded to Student Health Services.

4. Once the diagnosis of ADHD is confirmed by Student Counseling Services, an examination by a Student Health Services medical provider may then be scheduled.  The medical examination may include laboratory testing and an EKG.

5. When being treated for ADHD, an appointment with a Student Health Services medical provider is required every three months.

NOT previously diagnosed with ADHD:

1. Schedule an appointment with Creighton Student Counseling Services (402-280-2735).

2. A formal ADHD evaluation will be completed.  The fee for evaluation is $200. 

4. If the evaluation confirms the diagnosis of ADHD, results of the evaluation will be forwarded to Student Health Services and the student will be able to schedule a medical examination with Student Health Services.  The medical examination may include laboratory testing and an EKG.
