- patient's full name
- policiy holder's full name

- patient date of birth

Payment
All payments are due at time of service. For your convenience, we accept cash, checks, debit cards and major credit cards. You may make a payment over the telephone with a credit or debit card, by calling (402) 280-5059. Creighton Dental School is not a free clinic. We do not have a sliding fee scale.
Insurance and Medicaid
Creighton Dental School accepts most dental insurance policies, but not all.
We accept Nebraska and Iowa Medicaid.
Please bring your Insurance or Medicaid card/information with you to your appointment.
IMPORTANT:
It is patient's responsibility to verify benefits with his or her dental insurance provider. Please, call your insurance company to determine if you are covered for services provided at Creighton Dental Clinic.
It is the patient's responsibility to pay deductibles and co-payments at the time service is provided.
We will verify the patient's eligibility. Be prepared to pay in full at time of service if your eligibility cannot be determined or if you are receiving a non-covered service or if we are not a provider under your insurance plan.
