Commonly Used Forms

Depending on your particular circumstances, you may be asked by the CU Financial Aid Office to provide additional information. If this is the case, we will send you an email alert requesting it. Below are the most commonly requested forms we may ask you to submit. Many forms cqn be submitted on-line. Or you can print out the form you need, complete it and mail, fax, or bring it to the Creighton Financial Aid Office, 2500 California Plaza, Omaha, NE 68178-0062. We are located in ther Harper Center rm 2040. Our FAX is 402-280-2895. Oh, one more thing... don’t forget to sign any paper forms, or we have to mail it back for a signature. Most forms listed below are in PDF format. You must have the free Adobe  Acrobat  Reader loaded on you computer to view and print these forms. If you do not have Adobe Acrobat Reader, you may download the FREE application.

  ~      CU Verification worksheet 2009-10

 

 ~      Summer Aid Application 2009

            or On-line  summer aid application 2009.

 

 ~      Federal Work Study Agreement 2009-10

            or On-line  FWS agreement (submit this agreement only if you have been awarded and have accepted your FWS campus employment opportunity.)

 

 ~      I-9 Federal Employment Eligibility form is an on-line form that must be completed in person at time of hire with original documentation proving eligibility to work in the US. 

 

 ~      On-line Family Scholarship 

 

 ~      MET (Monthly Electronic Transfer). This payment program is administered through the Creighton Business office.

 

 ~      Parents’ Estimated Yearly Income for 2009

 

 ~      Appeal for Independent Status 2009-10

 


Tuition Remission forms for employees are available through the Creighton Human Relations department.

The Financial aid office may require that you and your parents submit copies of your federal tax returns and W2's.  If you can't find a copy of your tax return, you can get a free transcript of your tax return from the IRS by calling 1-800-829-1040 or by completing and sending this request form   #4506-T to the IRS.
