Depending on your particular circumstances, you may be asked by the CU Financial Aid Office to provide additional information. If this is the case, we will send you an email alert requesting it. Below are the most commonly requested forms we may ask you to submit. Print out the form you need, complete it and mail, fax, scan or bring it to the Creighton Financial Aid Office, 2500 California Plaza, Omaha, NE 68178-0062. On-line Federal Work Study agreement or paper version. Submit the FWS agreement only if you have been awarded and have accepted your FWS campus employment opportunity. Note that if you plan to work on campus, federal law requires that you complete a Federal W-4 tax withholding form and a Federal I-9 Employment Eligibility Verification form at time of hire. You will need to present, IN PERSON, to our staff an original, unexpired document that proves you are legally eligible to work in the US. Go here to view the I-9 form. The last page is a list a acceptable documents. You cannot complete the I-9 until you come to campus with your original document for us to witness. | |

Something new this year . . . .The US Department of Education requires the IRS tax return transcript to be submitted to the financial aid office rather than copies of the 1040 tax return. To request a transcript, go here. To order a transcript electronically, click on "Order a Transcript" under the number 3.
If you agreed to the data match ( IRS Data Retrieval process) with the IRS when you completed your FAFSA, and the match was successfully completed, you don't need to send us the tax transcript.
