Depending on your particular circumstances, you may be asked by the CU Financial Aid Office to provide additional information. If this is the case, we will send you an email alert requesting it. Below are the most commonly requested forms we may ask you to submit. Print out the form you need, complete it and mail, fax, scan or bring it to the Creighton Financial Aid Office, 2500 California Plaza, Omaha, NE 68178-0062. We are located in the Harper Center rm 2040. Our FAX is 402-280-2895. Please don’t forget to sign any paper forms, or we have to mail it back for a signature. Commonly used forms for aid year 2013 - 2014:
- Creighton Verification form 2013-14
- On-line Federal Work Study agreement Submit the FWS agreement only if you have been awarded and have accepted your FWS campus employment opportunity.
- Appeal for Independent Status for 2013-14
- Parents' Estimated Income for 2013
- Summer Aid Application for 2013
- Statement of non-filer of Federal Tax return
If you are selected for verification, the US Department of Education requires the IRS tax return transcript be submitted to the financial aid office rather than copies of the 1040 tax return. To request a tax return transcript, go here. If you agreed to the data match ( IRS Data Retrieval process) with the IRS when you completed your FAFSA, and the match was successfully completed, you don't need to send us the tax return transcript. We recommend that you use the Data Retrieval tool. It is easier and faster. (Not all filers are eligible to use the Data Retrieval.) The aforementioned process link will give you more information.
Also, if you completed your FAFSA before you submitted your taxes, you can go back to your online FAFSA and do a correction to retrieve and link your tax return to your FAFSA. Do this about 2 weeks after you have filed electronically or 8 weeks after you have submitted a paper tax return. |
