Address/Phone Number Changes
Address and/or phone number changes are managed by the employee online through Employee Self Service. To view and/or update your address or phone number, click here and you will be directed to MyCreighton. After you have logged on, click on the HR Self Service icon, then click on Employee Self Service. This will take you to the Personal Information option to view your data and make any necessary changes.
Add New Dependent
Go online at portal.adp.com or call the CU Solutions Center at 866-903-8216 within 31 days of the birth or adoption to add your child to the plans for which they need coverage. Also, to update Creighton's records, please complete an Employee Data Form and e-mail to humanresources@creighton.edu. In cases of adoption, you must provide a copy of finalized adoption papers to HR/Benefits.
Beneficiary Designation Changes
Log on to portal.adp.com or call the CU Solution Center at 866-903-8216 to elect or make changes to beneficary designations.

Dependents
Creighton's medical, dental, and vision plans allow coverage for dependent children to age 26, more than what is required by Healthcare Reform. Changes outside of open enrollment can be made due to a qualified status change.

Eligibility for Benefits
If you are a new employee, you may enroll in benefit plans within 31 days of your date of hire. When you enroll, the coverage you elect will begin the first of the month following your date of hire. New employees, click here to enroll in benefits.

Employment Status Change
Employees who work 20 hours or more per week will remain in a benefit eligible status. If work hours drop below 40 per week, vacation, sick and holiday accruals will change. A change in salary affects life insurance premium, long term disability premium and coverage, and retirement contribution/match.

Disability or Voluntary Life Insurance Coverage
If you wish to enroll in the Short-Term/Long-Term Disability plan or make changes to your life insurance after your new hire enrollment period has passed, underwriting approval will be required. Log on to portal.adp.com to choose your new coverage level. You will be asked to complete the required Evidence of Insurability (EOI) application. New Hire Haste Enrollment
In the case of an emergency where you need to use your coverage but have not yet received your new cards, please contact Human Resources at 280-2709 and we will work with our vendors to enroll you immediately.
Newly Married
Within 31 days of the event:
- Go online to portal.adp.com or call the CU Solution Center at 866-903-8216 to add your spouse and/or dependents to your benefit plans and update your beneficiary designations. To update Creighton's records, please complete an Employee Data Form and send to the Human Resources Department or e-mail a copy to humanresources@creighton.edu.
- Provide a copy of your Marriage Certificate to HR/Benefits.
- If your last name has changed, you must bring your new Social Security card to Human Resources.
- Both the W-4 and Direct Deposit forms are available on the HR website under Resources/Forms. Print, complete, and send via intercampus mail to the Payroll Department - Schneider Hall Bldg.
- Log on to Employee Self Service and make any address changes.
Qualified Status Change
Deductions for Medical, Dental, Vision, and other benefits are taken on a pre-tax basis. This means that you don't pay taxes on those dollars, and are thereby reducing your taxable income. Once you enroll in pre-tax benefits, either as a new hire or during Open Enrollment, your choices will remain in effect for the rest of the plan year unless you experience a "qualifed status change". Federal law sets specific rules about changes that can be made to benefit elections when premiums are paid on a pre-tax basis.
Examples of qualified status changes are:
- Change in legal marital status (marriage, divorce, annulment, death)
- Change in number of dependents (birth, adoption, death)
- Change in employment status that affects coverage (new/termination of employment or change in hours worked by a dependent spouse)
- Loss or gain of other coverage
Notification of your qualified status change MUST be made within 31 days of the event by logging on to portal.adp.com or by calling the Solution Center at 866-903-8216. If you do not request a change within 31 days of the event, the next opportunity to make changes will be at Open Enrollment to be effective the first day of following plan year.
The addition/deletion of a spouse or new dependent will be effective on the first of the month following the date of the qualifying event. Newborns will be retroactively effective on their date of birth. Proper documentation for qualifed status changes must be presented to CU Benefits. 403(b) Retirement Plan Eligibility
Employees are eligible to participate in the Retirement Savings Plan as of their date of hire. See the New Hire Benefits Enrollment Guide for detailed instructions.
403(b) Retirement Plan Match
You will become eligible for Creighton's match when you have completed two years of service. If you are enrolled and contributing to the plan prior to your match eligibility date , the match will begin automatically according to the match schedule. If you are not enrolled, you need to enroll and elect the percentage you wish to contribute. To take full advantage of the 6% match, you will need to contribute 5%.
