New Hires / New to CCAP/HSA:
New Hires Enrolled in the CCAP Health Plan
All employees enrolled in the Creighton Consumer Account Health Plan (CCAP) must complete their SimplyWell Health Risk Questionnaire and Health Screening to receive employer contribution money into their Health Savings Account with First National Bank (FNB).
Requirements for employer contributions
1. You will choose a date for your health screening when completing your health risk questionnaire. Health Screenings scheduled times are 7:00 am – 9:00 am Room 220 KFC. See screening schedule dates:
March 18, 2009 | June 17, 2009 |
April 15, 2009 | July 16, 2009 |
May 20, 2009 | August 19, 2009 |
2. To complete the Health Risk Questionnaire, go to www.simplywell.com. The enrollment guide is available on the Wellness Program website (go to Faculty/Staff web page) or contact Dawn Obermiller ext. 5721 with questions.
Employer Contribution Amounts
The employer contributions paid into the HSA bank account on behalf of the employee is pro-rated based on the following schedule:

Upon completion of the Health Risk Questionnaire and screening, employer contribution deposits will be deposited on the next available payroll.
Instructions on Opening the HSA with First National Bank
Steps:
To establish the Health Savings Account through First National Bank. Please log onto the following website within 31 days of your employment.
If you have already enrolled in a Health Savings Account through First National Bank, do not enroll again as this will open a duplicate account. If you are unsure if you have enrolled, please contact Amanda Hopkins at FNB Customer Service 402-633-2427.
If you are a new hire, if you have submitted your application but are unsure the account was established, please do not try again. Contact Amanda Hopkins - FNB Customer Service 402-633-2427 to check on the status of your account.
If you do not have a drivers license, First National Bank requires a State ID which you can obtain from the Department of Motor Vehicles. The Department of Motor Vehicles requires 2 forms of ID, which can include foreign passport and I-94, the visa is not a valid form of ID.
Once you have completed this enrollment process, First National Bank will issue you a debit card for your account and Creighton University can send the employer monies to your account.
