Application/Proposal Forms
Prior to studying or traveling abroad, all undergraduate and graduate/professional students must complete one of the following forms to notify the OIP of their plans and obtain approval for studying/traveling abroad.

NOTE: Spaces on Encuentro Dominicano, FLPA Summer Travel Courses, Affiliate Programs Limerick & Glasgow, Bilateral Exchange Programs Sogang & Sophia, and Multilateral Exchange ISEP Programs fill early. For these programs, students are encouraged to submit their Application/Proposal to the OIP as early as possible.
Applications & Proposals to Study Abroad are accepted anytime and may be submitted before the Priority Deadline indicated above. Applications/Proposals will NOT be accepted after the Final Deadline indicated. If a deadline falls on a weekend or holidy, the Application/Proposal will be due the following business day by 4:30p.m.

This form is for all Creighton undergraduate and graduate students planning to participate on a Creighton Faculty-Led Summer Program Abroad (FLPA). | |
This form is for any Creighton undergradute student planning to study abroad on any program (except Encuentro or a FLPA course) during the summer, fall semester, spring semester, or full academic year. You must attend a Study Abroad 101 session or complete the online tutorial before your Proposal will be reviewed. | |
Graduate & Professional Student Travel Abroad Information Sheet | This form is for any graduate or professional school student traveling abroad to fulfill a degree requirement through volunteer, research, externship, internship, rotation, etc. (NOTE: students particpating on FLPA, ILAC or Project CURA trips do not need to complete this form). This form must be compled and returned to the OIP at least 2-6 months prior to departure, or earlier. |
