This form is for Creighton undergraduate students planning to participate on Creighton's semester-long program in the Dominican Republic. Students from other institutions interested in this program should contact studyabroadadvisor@creighton.edu to request an application form.
Faculty-Led Program Abroad Application for Participation
This form is for undergraduate and graduate students planning to participate on a Creighton faculty-led summer trip. Summer 2010 Applications can be picked up in the Office of International Programs (M-F, 8:00am-4:30pm)
CU Proposal to Study Abroad
This form is for all other undergraduate students planning to participate on any study abroad program including, affiliate programs, bilateral exchange programs, multilateral exchcange programs, partner programs, and associate programs.
Graduate & Professional Student Program Abroad Information Sheet (COMING SOON)
This form is for any Creighton graduate or professional student traveling abroad to meet a requirement torwards the Creighton degree through study, research, volunteer service, internship, or work abroad.
Pre-Departure Forms
Prior to departure every student will be required to submit the following forms and documents to the OIP. These forms may be obtained from the Study Abroad Coordinator. A detailed 'Forms Checklist' will be provided to you when receiving these forms. Students who do not have complete files prior to departure will not be eligible to receive "final study abroad approval."
