Prior to studying or traveling abroad, all undergraduate and graduate/professional students must complete one of the following forms to notify the OIP of their plans and obtain approval for studying abroad.
SUBMISSION DEADLINES
Faculty-Led Summer Programs Abroad (FLPA's)
Priority Deadline: December 1st; Final Deadline: January 15th
Encuentro Dominicano FALL Semester Participation
Priority Deadline: December 1st; Final Deadline: February 1st
Encuentro Dominicano SPRING Semester Participation
Priority Deadline: May 1st; Final Deadline: August 1st
Affiliate, Exchange, Partner, & Associate Programs SUMMER/FALL/FULL-YEAR PARTICIPATION
Priority Deadline: December 1st; Final Deadline: March 1st
Affiliate, Exchange, Partner, & Associate Programs SPRING SEMESTER PARTICIPATION
Priority Deadline: May 1st; Final Deadline: October 1st
NOTE: Spaces on Encuentro Dominicano, FLPA Summer Travel Courses, Affiliate Programs Limerick & Glasgow, Bilateral Exchange Programs Sogang & Sophia, and Multilateral Exchange ISEP Programs fill early. For these programs, students are encouraged to submit their Application/Proposal to the OIP as early as possible.
Applications & Proposals are accepted anytime and may be submitted before the Priority Deadline indicated above. Applications/Proposals will NOT be accepted after the Final Deadline indicated. If a deadline falls on a weekend or holidy, the Application/Proposal will be due the following business day by 4:30p.m.

This form is for all Creighton undergraduate and graduate students planning to participate on a Creighton Faculty-Led Summer Program Abroad (FLPA). | |
This form is for any Creighton undergradute student planning to study abroad on any program (except Encuentro or a FLPA course) during the summer, fall semester, spring semester, or full academic year. You must attend a Study Abroad 101 session or complete the online tutorial before your Proposal will be reviewed. | |
Graduate & Professional Student Travel Abroad Information Sheet | Coming Soon! This form is for any graduate or professional school student traveling abroad to fulfill a degree requirement through volunteer, research, externship, internship, rotation, etc. (NOTE: students particpating on FLPA, ILAC or Project CURA trips do not need to complete this form). This form must be compled at least 4 weeks prior to departure, or earlier. |
Pre-Departure Forms for ALL STUDENTS
Prior to departure every undergraduate and graduate/professional student will be required to submit the following forms and documents to the OIP. These forms may be obtained from the Study Abroad Coordinator. Students who do not have complete files prior to departure will not be eligible to receive academic credit or financial aid for their experience overseas.
Original Signed Forms should be returned to the OIP by the "Forms Deadline" (faxes and scans cannot be accepted):
- December 1 for students traveling abroad during the SPRING SEMESTER
- May 1 for students traveling abroad during the SUMMER, FALL SEMESTER, or ACADEMIC YEAR.

Assumption of Risk & Release Waiver Form |
Read and Sign this form. If you are under the age of 19, you will also need the signature of a parent/guardian. |
Health Disclosure Form | Complete all sections of this form; information pertains to your health history, allergies, etc. |
Medical Approval Form | This form must be completed and signed by a physician. Schedule a travel physical as soon as possible (no earlier than 6 months prior to your departure) with CU Student Health or another healthcare provider. |
Copy of Immunization History | Obtain a copy of your immunization history from your current physician or Creighton Student Health. Your name must appear on the record or it will not be accepted. |
Consent for Emergency Medical Treatment Form | List TWO emergency contacts on this form. |
Flight Information Form | Once you have made flight arrangements, complete this form. Please register your trip with the U.S. State Department (you will need yoru passport #, emergency contacts in the U.S., and in-country contact information. |
Copy of Flight Itinerary | Print or copy your complete flight itinerary. |
Travel Insurance Enrollment Form | Complete the appropriate enrollment form, as instructed on the form. |
