All Creighton faculty, staff, and students traveling abroad under the auspices of Creighton University are required to purchase the supplemental travel insurance administered by the OIP. Individuals who are traveling abroad for personal reasons are also welcome to purchase the travel insurance. All applicants must be 14 years of age (or older) at the time of application (proof of age required).
All travelers should maintain primary health insurance coverage while abroad. The travel insurance offered by the OIP is a supplemental (secondary) policy. Costs and benefits differ by level of coverage. Coverage levels available differ for each group below. To ensure that you complete the correct Travel Insurance Enrollment Form, review the following information very carefully.

INSURANCE ENROLLMENT FORMS
STUDY ABROAD PARTICIPANTS
All study abroad students (summer, semester, and full-year participants) will automatically receive the Travel Insurance Enrollment Form as part of the "Pre-Departure Forms Packet." Please contact the OIP at 402-280-2221 for additional information.
FACULTY-LED SUMMER PROGRAMS ABROAD (FLPA) STUDENTS
Students participating in a Creighton Faculty Led Summer Program Abroad will receive the Travel Insurance Enrollment Form as part of their "Pre-Departure Forms Packet." Please contact the OIP at 402-280-2221 for additional information.
ILAC PARTICIPANTS
Individuals who are participating/volunteering with the Institute for Latin American Concern (ILAC) will receive the Travel Insurance Enrollment Form directly from the ILAC office. Please contact ILAC at 402-280-3179 for additional information.
PROJECT CURA PARTICIPANTS
Creighton School of Medicine students and volunteers participating on a Project CURA summer trip, will receive the Travel Insurance Enrollment Form directly from the SOM.
CREIGHTON FACULTY AND STAFF
This form is for Creighton faculty or staff who are traveling abroad for any purpose including (but not limited to) research, conferences, volunteering, or personal travel.
GRADUATE AND PROFESSIONAL SCHOOL STUDENTS
This form is for any graduate or professional school student who is traveling abroad for any reason including (but not limited to) study, internships, research, externships, rotations, or conference attendance. (Project CURA students do NOT complete this form).
FRIENDS AND FAMILY OF CREIGHTON STUDENTS, STAFF, & FACULTY
This form is for friends and family members of Creighton students, staff, or faculty who wish to enroll in the travel insurance. (Note: Applicants must be 14 years of age or older at the time of application; proof of age required).
