2010 Spring and Fall Break Coordinator Online Application

1) Read the Coordinator Role and Responsibilities

2) Download the “Coordinator Essay Questions,” complete and print.

3) Please fill out the online portion (below) of the application and hit the “submit” button. You will need your insurance info ready for the emergency form portion.  (Complications are possible when using Safari or Chrome. Internet Explorer or Firefox are recommended.)

4) After you have submitted the online portion of your application you will receive a confirmation email. In order to complete the application process...

You need to print and/or bring the following documents/items to the CCSJ Office in the Harper Center (2067):
-Completed Essay Questions
-Your payment of $180 (we accept cash, check or credit card in the office). 
-You will need to sign up for an interview in our office on or before Oct. 12 (interviews are 15 minutes in our office; no need to dress up) (applications will be accepted if turned in by Oct. 27th if there is a possible interview time available Oct. 28-30)

**If you are under 19, download the "Under 19 Parent Consent and Release Form" document. You must have a parent or guardian sign it and send or fax it to the CCSJ (402-280-1259). **

DRIVING, NO-ALCOHOL, CELL PHONE, AND REFUND AGREEMENT

  1. As a driver (if applicable), I bear in mind that I am responsible for the safety of my passengers and promise to drive safely, make sure everyone is wearing their seat belt, and observe the local traffic laws and speed limits.
  2. At no time and under no circumstances will I consume alcoholic beverages or other illegal substances for the duration of the 2010 Spring/Fall Break Service Trip. This includes from the time I leave Creighton University until the time I return, even I am of legal age to drink.
  3. I understand that for the sake of simplicity and community, the F/SBST program asks participants to leave cell phones at home (Coordinators will have cell phones to let loved ones know of safe arrival and for emergencies). If I choose to bring a cell phone with me, I will leave it off and packed in my bag unless an emergency arises.
  4. If you are accepted and you decide not to participate in the service trip after the final drop day, your fees will be non-refundable.
