SearchFall Break Service Trips (FBST)
Steps to apply for a Fall Break Service Trip (October 16/17th-24th, 2009):
A. APPLICATION PROCESS (will be up shortly, in the next few days)
1) Download the “FBST Essay Questions,” complete and print.
(If you have gone on a Fall or Spring Break Service Trip before, you may complete these Returner Essay Questions).
2) Fill out the online application and hit the “submit” button.
*You will need your insurance info ready for the emergency form portion.
*You will be asked for 2 references
*You will be ranking several items, so plan about 20 minutes to complete the online app.
*(Complications are possible when using Safari. Internet Explorer or Firefox are recommended.)
3) After you have submitted your application online, you will receive a confirmation email to let you know we received your application.
B. DATES TO REMEMBER
Sept. 3 Thurs @ 9 pm or Tues. Sept. 8 @ noon FBST Information Sessions (both are the same, and both are held in the CCSJ Office)
FBST Scholarships Apps Due (asap): No later than Mon. Sept. 7, by midnight
Sept. 9 Wed. by 4 pm FBST Applications Due in the CCSJ Office (includes online application, essays, payment and signing up for your interview)
Sept. 11-15 FBST Interviews CCSJ Office
Sept. 21 Tues. FBST List Posted

Trip Dates: Friday October 16 or Saturday October 17 - Sat, October 24, 2009
C. PAYMENT
The student payment for the trip is $230 (we accept cash, check, or credit card--if you are only applying for 1/2 week trip, $115 due, if you are applying for either you will pay $230 and will be refunded if placed on 1/2 trip).
** Limited need-based scholarships are available. Download the “Scholarship Application” and send it to Kelly Orbik (korbik@creighton.edu). Scholarship applications are due Sept. 7th. **
Full refunds available until Friday September 18, 2009. After this date $125 of the whole week and $60 of the 1/2 week cost is non-refundable. If you are accepted and you decide not to participate in the trip after September 24, 2009, your entire fee will be non-refundable. This is necessary because the CCSJ allocates site payments based on your acceptance to the program.
D. WHAT YOU NEED TO DO
You need to print and/or bring the following documents/items to the CCSJ Office in the Harper Center (2067):
1. Completed FBST Essay Questions (See A)
2. Sign up for an interview in our office on or before Sept. 9 (interviews are 15 minutes at the CCSJ; no need to dress up)
3. Your payment for the trip
4. **If you are under 19, download the "Under 19 Parent Consent and Release Form" document. You must have a parent or guardian sign it and send or fax it to the CCSJ (402-280-1259). **
Applications are due Sept. 9, any application received after will be marked late, but still accepted as long as we are able to schedule an interview.
Check out all the great sites we are going to this year. All applications are to the general Fall Break Service Trips Program, not for a specific site. We will take into consideration your preferences but offer no guarantees of where you will be placed.
