Applications

Points of information about the SBST 2010 Program
1. For the sake of simplicity and group unity, each student will sign an agreement to not consume alcohol, bring an IPod (or other personal music player), use a personal cell phone while on the trip (the group will have a cell number they can give to family/friends for emergencies) and will abide by Creighton’s Handbook while on a trip.
2. The list of participant placements will be posted by Monday, Jan 25 in the CCSJ office. We will also e-mail everyone.
3. Our coordinators are of all ages, sophomores-seniors and a few graduate students.

1st General Meeting:        Thurs., Jan. 28             9:00-10:30 p.m.          Harper Center Ballroom

** Limited need-based scholarships are available. Download the “Scholarship Application” and send it to Kelly Orbik (korbik@creighton.edu). Scholarship applications are due Thursday. Nov. 19 (before applications!). **  

1) Download and complete the “SBST Essay Questions" (bring them with you to the CCSJ or e-mail them to korbik@creighton.edu before Nov. 24)

2) The online portion of the Application will be available after Nov. 17th, please fill out this part and hit the “submit” button. You will need your insurance info ready for the emergency form portion. You will be ranking several items, so plan about 20 minutes to complete this portion. (Complications are possible when using Safari. Internet Explorer or Firefox are recommended) [After you have submitted your application online, you will receive a confirmation email to let you know we received the online portion of your application]

An on-time/complete service trips application will include the follwing items:
-online application (you will know this is done if you get a confirmation e-mail)
-Completed SBST Essay Questions 
-Your payment of $250 (We accept cash, check or credit card)
(if you are ONLY applying for a 1/2 week trip, $125 is due, if you are applying for either you will pay $250 and will be refunded if placed on a 1/2 week trip)
(if you do not pay with a credit card at the time of your online application, you can return to pay here).
-You will need to sign up for an interview in our office on or before Nov. 24 (interviews are 15 minutes in our office;  no need to dress up)
**If you are under 19, download the "Under 19 Parent Consent and Release Form" document. You must have a parent or guardian sign it and send or fax it to the CCSJ (402-280-1259). **

Check out all the great sites we are going to this year. All applications are to the general Spring Break Service Trips Program, not for a specific site.  We will take into consideration your preferences but offer no guarantees of where you will be placed.

Applications are due Nov. 24, 2009 any application received after will be marked late, but still accepted as long as we are able to schedule an interview.

Full refunds available until Friday December 18, 2009. After this date $150 of the whole week and $70 of the 1/2 week cost is non-refundable. If you are accepted and you decide not to participate in the trip after Jan. 25, 2010, your entire fee will be non-refundable. This is necessary because the CCSJ allocates site payments based on your acceptance to the program. If you pay by credit card, refunds after 60 days will have a processing fee deducted.
