Print this form, read and sign (links below; pdf or Word). All eligible program participants under the age of 22, are required to have this form signed by a parent/guardian. Once complete, the form (with original signatures from both the student and parent/guardian) should be dropped off to the GEO front desk (Creighton Hall 324) or mailed to the address below. If it is not possible to return the completed, original form by mail or in-person, the form can be emailed directly to firstname.lastname@example.org (email must be received from the parent/guardian signing the form). ELECTRONIC SIGNATURES ARE NOT ACCEPTED.

Choose someone who can speak about your interests, personal characteristics, and/or abilities that would make you a good fit for the Encuentro program. Please ask your reference to return the form to you in a sealed envelope and include it with the other supporting documentation you submit to GEO.
