Summer Payroll Directives for Instructors:
Please fill out the following forms, as applicable:
1. W-4NA
(If you live out of state (Nebraska), you will need to fill out a W-4NA
Withholding Form unless you have taken a vow of poverty.)
2. Payment Information Form
(Instructors are required to complete this form for payroll.)
3. W-9 Request for Taxpayer Identification Number & Certification
4. Direct Deposit Authorization Form
(Print and complete the form and attach a VOIDED CHECK from your bank)
