International Arrival Reporting Form for all Community Members
In an effort to help keep our Creighton campus members safe and healthy, all students, faculty and staff are asked to register their arrival in Nebraska from any international origin starting June 1, 2020. If you are currently traveling or planning travel to occur before Aug. 31, 2020 please register your arrival. This is extremely important for follow-up from the Student Health Education and Compliance Office and Douglas County Health Department in the event of further spread of the virus. To register your arrival please fill out this International Arrival Report Form.
Travelers returning to other states and are advised to review and follow guidelines established by regional Health and Human Services Departments.
University-sponsored travel has been canceled until further notice.
Creighton strongly recommends that all travelers (faculty, staff or students) refrain from making travel arrangements until the University has officially announced the resumption of Creighton-sponsored travel programs.
