Creighton University's Volunteer Acknowledgment Form should be used for anyone who wishes to volunteer at Creighton University. There may be volunteers in a number of different locations throughout the University, but most often, these volunteers will work in labs at Creighton. For these volunteers, the Volunteer Committee has also developed and adopted the Laboratory Personnel Safety Check List for Volunteers.

Completed copies of the Volunteer Acknowledgement Form and the Laboratory Personnel Safety Check List for Volunteers should be returned to Katie Booton, University Risk Manager, Linn Building, Room 123, or via PDF at kbooton@creighton.edu. If you are volunteering in a lab at Creighton University, you are required to complete the online Lab Safety Training.
