Applying for our CAP Fellowship
The Creighton-Phoenix Child & Adolescent Psychiatry Fellowship participates in the Electronic Residency Application Service (ERAS), which is supported by the Association of American Medical Colleges (AAMC). Applications will only be accepted through ERAS.
Requirements
- Three (3) years of General Adult Psychiatry Training at an ACGME-accredited institution.
- Three letters of reference, including one from the program director
- General (Adult) Program Director’s Attestation Form for Child and Adolescent Psychiatry (CAP) Fellowship Eligibility (to be filled out by Program Director and included with their letter of reference
- Program Director verification of completed experiences
- Eligibility for license to practice medicine in Arizona
- Step 3 successfully completed
- J-1 visas accepted
- Deadline for applications is November 1
