The eligibility form MUST be filled out and signed by your supervising dentist. You will not be admitted to the course without both items, and no refund will be given. One year of clinical work experience as a dental assistant is required.</p><p><strong>Cost:</strong> $450</p><p><strong>Instructors:</strong> Lisa Kucera, RDH; Laura Grabow, RDH</p><p><strong>Method of Teaching:</strong> Lecture and hands-on skills lab. Day 1 of the program will be delivered in-person with audience participation.
