Obtaining an ID

Welcome to Creighton University Card Services! Below are the steps for requesting a first time campus ID card through email.

1. Download the required forms. All forms are in PDF format and will require Acrobat Reader.
   • Application (only required for distance students)
   • ID Terms and Conditions
2. Acquire a photograph of yourself for the ID card. Only photos that meet the specifications below will be accepted:
   • Submit a color photo, taken in last 6 months
   • Use a clear image of your face. Do not use filters commonly used on social media.
   • Have someone else take your photo. No selfies.
   • Remove hats and sunglasses.
   • Use a plain white or off-white background.
   • Use sufficient lighting so there are no shadows on your face.
