How to Apply Surgery
CUSOM Phoenix General Surgery Residency participates in the National Resident Matching Program (NRMP). Candidates are judged based on fit (mission alignment and goals), academic background, personal statements, letters of recommendation and interviews. The Department strictly follows EEOC guidelines. Program Leadership review all applications that meet our criteria.
We accept eight categorical interns and up to 12 preliminary interns. Our program does work with the CUSOM Phoenix podiatry program which accepts two PGY1 podiatric interns separately.
All our positions are filled through the National Resident Matching Program (NRMP)

Submitting an Application
All applications must be submitted via the ERAS program. Applications are accepted from September 15th to November 30th. Please note: Creighton does not sponsor visas of any kind (i.e., J1, J2, H1, H2, H3, or F1). We do consider MD, DO, IMG and FMG applicants. We do consider (and have matched) current preliminary residents for categorical positions.
The following must be submitted for your application to be considered complete:
- Application through ERAS
- Curriculum Vitae
- Three letters of recommendation: Must include surgeons who can evaluate your performance during a general surgery clerkship.
- Copy of a transcript from your medical school.
- USMLE scores: step 1 (if available) and 2.
- Surgery Shelf Exam score
- Maximum of 2 years out of medical school.
- Foreign Medical School/Graduates must submit a current ECFMG and CSA certificate. You will find the most current ECFMG requirements at the Educational Commission for Foreign Medical Graduates.
- Selected categorical applicants are invited to interview on one of SIX formal interview days. We interview candidates for preliminary positions separately. This year's interview dates are pending. The agenda includes a briefing by the Chairman and Program Director, a tour of the hospital, interviews with the PD, Chair, faculty, and Residents. We hope to be in person this coming year but did successfully use a virtual format last year. Please check back for updates.
- For the 2022 ERAS® cycle, our program encourages all applicants to complete a supplemental ERAS application, offered through the AAMC’s ERAS program. Applicants also will be required to complete the MyERAS application.
- The additional information collected will help our program staff learn more about applicants’ medical education journey, including more meaningful information about work, volunteer, and research experiences, and will help us better identify applicants who fit our program environment and mission.
- This supplemental ERAS application will be delivered on a survey platform that is separate from the MyERAS application and must be completed between September 1-30.
