IPH Community Health Navigator Program Underway

The Institute for Population Health (IPH) launched its Community Health Navigator (CHN) program in March at Creighton & Community at the Highlander, Jabari Wright, IPH community health navigator, prioritized building referral pathways through partnerships with community organizations, including Community Relay, where Creighton & Community is listed. 

The program supports community members through referrals, community events and self-referrals.

The program is designed to connect individuals and families living in the Highlander neighborhood and greater North Omaha community to resources that address social determinants of health and support overall well-being. Services include:

  • Referrals to social services and community-based programs
  • Assistance with enrolling in health insurance programs, including completing Medicaid and Medicare applications
  • Support for utilities, housing-related resources and other basic needs
  • Guidance navigating healthcare and community systems
  • Connection with appropriate service providers

To meet growing demand, Creighton & Community plans to add a second CHN. Identified priority needs among community members include housing, employment and transportation services

Walk-in and prescheduled appointments are available Monday-Friday from 9 a.m.-5 p.m. Evening and weekend appointments are available by calling (402) 280-5875.
