The Center for Medication Therapy Management provides comprehensive medication management to improve healthcare outcomes and limit expenditures. Collaborating within integrated healthcare systems among community partners, the Center engages with patients and empowers them to take an active role in their health. The Center also enables student pharmacists to gain clinical knowledge and expertise through the provision of medication therapy management services.
Medication Therapy Management (MTM) optimizes therapeutic outcomes for individual patients. The Center’s MTM program is dedicated to providing annual comprehensive medication reviews (CMRs) for Medicare beneficiaries.
A CMR is a one-on-one discussion with a patient in which the pharmacist reviews all of a patient’s medications including prescription and nonprescription medications, herbal products and dietary supplements. During a CMR, drug therapy issues such as drug interactions, nonadherence and gaps in patient care are identified and addressed with the patient. The discussion also provides a unique opportunity for patients to ask questions or address concerns about their medications.
At the conclusion of the CMR, a personal medication list (PML) and medication action plan (MAP), is generated for the patient.
The Center for Medication Therapy Management has partnered with AdhereHealth, a nationally renowned provider of MTM services. Through this partnership, the Center will serve as a provider group for the delivery of MTM services to contracted CMS-regulated health plans.
Students participating in the Center’s MTM rotation gain competency in identifying, preventing and resolving medication-related problems, as well as effectively communicating with patients and healthcare providers, to optimize therapeutic outcomes.
In this advanced pharmacy practice experience (APPE), students develop an array of skills, including comprehensive medication review, patient education, written and verbal communication skills and assessment and documentation of services and therapy.
