Wellness programs focus on the individual, medical practice, organizations and the broader health care system. These interactive layers all play a part in our ability to maintain our physical, emotional, spiritual, intellectual, financial, environmental and multicultural wellness.

This form is rooted in Cura Personalis “care for the person” or “personal care” and provides another way for us to access support for ourselves and/or extend care to a member of our community.
