The Chronic Disease Intervention Program is a tool used to teach individuals how to self-manage chronic disease and increase knowledge about these chronic conditions. Self-management relates to the tasks that an individual must undertake to live well with one or more chronic conditions. The self-management program is designed to give participants skills to coordinate all the things needed to manage their health and not to conflict with existing treatment.
The content of the workshop will be the result of group discussion in which the participants discuss which areas are most useful to them. Subjects covered over 4 weeks may include:
